What the official deficiency says
(a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. (1) Disinfectants, cleaning solutions, and poisonous substances shall be stored in areas separate from food supplies as specified in Section 87555, General Food Service Requirements. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview, & record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPA observed R1’s physical it states the resident’s safety would be at risk if they had access to disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items. LPA observed laundry detergent & bleach above the washer to be accessible, a knife in an unlocked kitchen drawer, cleaning products in and unlocked cabinet under the sink, various tools in an unlocked drawer in the kitchen, scissors in kitchen drawer, antifungal cream in an unlocked drawer & toilet bowl cleaner under the sink in the hallway bathroom.

POC Due Date: 10/20/2025 Plan of Correction Administrator will provide pictures that items have been removed and relocated in a locked and secure area.

87555 General Food Service Requirements (b) The following food service requirements shall apply: (8) All food shall be of good quality. Commercial foods shall be approved by appropriate federal, state and local authorities. Food in damaged containers shall not be accepted, used or retained. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPA observed strawberries & zucchini to have mold. LPA observed bacon and sausage stored in plastic baggy with label or date. LPA observe expired macaroni salad in refrigerator.
