What the official deficiency says

Postural Supports. Bed rails that extend the entire length of the bed are prohibited except for residents who are currently receiving hospice care and have a hospice care plan that specifies the need for full bed rails. This requirement is not met as evidenced by: Deficient Practice Statement Based on LPA's observation, the licensee did not comply with the section cited above by utilizing full bed rails for R1 and R3 without Physician's order, which poses/posed a potential health, safety or personal rights risk to persons in care.

Official plan of correction

POC Due Date: 01/28/2026 Plan of Correction Administrator agreed to remove the full bedrail and provide a picture by the POC. The Administrator also agreed to provide Postural Support training to all staff members and submit training materials to LPA by POC due date.

Personal Accommodations and Services: d) The following space and safety provisions shall apply to all facilities: 6) All outdoor and indoor passageways and stairways shall be kept free of obstruction. This requirement is not met as evidenced by: Deficient Practice Statement Based on LPAs observation, during visit on 1/30/2025, the licensee did not comply with the section cited above to ensure the emergency exit door/passageway in room # 4 was free of obstruction, which poses an immediate health and safety risk to persons in care.
