This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. Based on observation the stove, refrigerator and kitchen cabinets in the kitchen was dirty with oil stains all over, which poses/posed a potential health, safety or personal rights risk to persons in care.

(5) The licensee shall assist residents with self-administered medications as needed. This requirement is not met as evidence by: Based on observation administrator did not ensure that R1's medications are listed in the MAR. R1's Folic Acid 1 mg and Aspirin EC 81 MG were present at the facility / prescribed by the doctor, but not listed / recorded on the MAR.

(h) The following requirements shall apply to medications which are centrally stored: (4) All centrally stored medications shall be labeled and maintained in compliance with state and federal laws. No persons other than the dispensing pharmacist shall alter a prescription label. This requirement is not met as evidenced by: Deficient Practice Statement During medication review, LPA observed Resident #1 had Vitamin D3 and Calcium D3 medications which were not labeled. LPA also observed Resident #2 had " Kirkland " Aspirin which was not labeled. This poses a potential health, safety, and/or personal rights risk to the residents in care.
