Verification of scheduled training with the trainer’s credentials will need to be submitted by 4/25/2022 and completion of training must be submitted no later than 5/13/2022. 87303 Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement was not met as evidenced by: Licensee failed to keep the carpet in the unit of R1 clean and well kept. During the investigation LPA observed the carpet to have multiple stains in various areas. This poses as a potential health and safety riske to the residents in care.
