What the official deficiency says

Requirements (a) Each licensee shall furnish to the licensing agency such reports... (1) A written report shall be submitted to the licensing agency and to the person... ... any of the events specified in (A), (B) & (D)... This requirement is not met as evidenced by: Based on interviews and record reviews, conducted by LPA, the licensee did not comply with the section cited above by failing to notify CCLD regarding R1's hospitalization on 02/26/26 and R3's on 02/12/26, which poses a potential health and safety risk to persons in care.

Licensee shall ensure a written report is submitted to the licensing agency and to the person responsible for the resident within seven (7) days of the occurrence of any of the events. R1's & R3's incident reports shall be submitted to LPA by POC date.

What the official deficiency says

(c)The facility shall employ, and the administrator shall schedule, a sufficient number of staff members to do all of the following: (3) Ensure that at least one staff member who has cardiopulmonary resuscitation (CPR) training and first aid training is on duty and on the premises at all times. This paragraph shall not be construed to require staff to provide CPR. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview and record review, the licensee did not comply with the section cited above in three (3) out of five (5) staff member which poses a potential health, safety or personal rights risk to persons in care.

POC Due Date: 08/06/2024 Plan of Correction Licensee will make sure to have a staff with CPR and First Aid training on duty and on the premises at all times. Licensee will provide a copy of CPR/First Aid certification for all three (3) staff by POC due date.
