87507 Admission Agreements (f) The licensee shall comply with all applicable terms and conditions set forth in the admission agreement, including all modifications and attachments. This requirement is not met as evidenced by: Based on interview and record review the licensee did not comply with the section cited above as R1 was charged for tray delivery service and standby shower assistance when the services were not provided to R1 which posed a potential personal rights risk to clients in care.

This requirement is not met as evidenced by: Based on observation, interview, and record review the licensee did not comply with the section cited above as R1's room was not cleaned, laundry was not done, and food was not delivered on time which posed a potential personal rights risk to clients in care.

1569.655 (a) If a licensee of a residential care facility for the elderly increases the rates of fees for residents...the licensee shall provide no less than 90 days' prior written notice...the amount of the increase...This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above when they issued a general rate increase with less than 90 days’ notice, which posed a potential personal rights risk to residents in care.

This requirement is not met as evidenced by: Based on interviews, the licensee did not comply with the section cited above. Staff did not obtain timely medical attention for R1 when R1 fell and complained of hip and back pain, which posed an immediate health and safety risk to residents in care.
