What the official deficiency says
(a) All facilities shall maintain a fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. Prior to accepting or retaining any of the following types of persons, the applicant or licensee shall notify the licensing agency and obtain an appropriate fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. Medical assessments/physician’s report for Resident #1 (R1) and Resident #2 (R2) revealed residents are non-ambulatory. Register of Residents revealed R1 lived in room 11 and R2 lived in room 14. Fire Safety Inspection revealed rooms 11 and 14 are for ambulatory residents only.

POC Due Date: 06/05/2026 Plan of Correction The Licensee will submit a plan of correction that includes how the Licensee will ensure the safety of the residents until it comes in compliance, training, and email medical assessments/physcian's report of the residents from Santa Monica Home & Care #1 - #4 (connected facilities) to regina.cloyd@dss.ca.gov by the POC due date.
