(a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement is not met as evidenced by: Deficient Practice Statement Based on [(observation), the licensee did not comply with the section cited above in one (1) out of one (1) stove which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 11/14/2025 Plan of Correction LPA and Staff two, Rachel Tuazon, have agreed to have the stove to be repaired or replaced on or prior to POC due date. S2 has confirmed that the facility will forward one (1) video evidence of the outcome (stove to be lit by included knob, with child safety knob cover) to LPA at MARIO.LEON@DSS.CA.GOV

(c) All window screens shall be clean and maintained in good repair. This requirement is not met as evidenced by: Deficient Practice Statement Based on Observation, the licensee did not comply with the section cited above in four (4) out of four (4) screens to be in disrepair, or completely missing (kitchen window) and/or having dirty window sills which poses/posed a potential health, safety or personal rights risk to persons in care.
