What the official deficiency says

(C) Clean linen, including blankets, bedspreads, top bed sheets, bottom bed sheets, pillow cases, mattress pads, bath towels, hand towels and wash cloths. The quantity shall be sufficient to permit changing at least once per week or more often when indicated to ensure that clean linen is in use by residents at all times. The linen shall be in good repair. The use of common wash cloths and towels shall be prohibited. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation , the licensee did not comply with the section cited above in 2 out of 6 residents' beds did not contain a top and bottom sheet and 1 out of 6 resident beds did not contain an unstained mattress cover which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 07/28/2026 Plan of Correction By POC due date Administrator will send a photo to LPA's email showing the beds of R1 and R2 with top and bottom sheets on them and with a replacement mattress cover on R2's bed.POC cleared during today's visit. Administrator put new sheets on R1's and R2's beds and replaced R2's mattress cover during LPA's visit.

What the official deficiency says

(3) A written order from a physician indicating the need for postural support shall be maintained in the resident’s record. The licensing agency shall be authorized to require other additional documentation if needed to verify the order. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, LPA observed that Resident #1 (R1’s) file did not have a written physician’s order for a bed rail which poses a potential health, safety or personal rights risk to persons in care.
