What the official deficiency says

(c)The facility shall employ, and the administrator shall schedule, a sufficient number of staff members to do all of the following: (3) Ensure that at least one staff member who has cardiopulmonary resuscitation (CPR) training and first aid training is on duty and on the premises at all times. This paragraph shall not be construed to require staff to provide CPR. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited as 3 out of 4 staff files ( S1, S3, S4) reviewed did not contain current First Aid training.

This requirement is not met as evidenced by: (e) Water supplies and plumbing fixtures shall be maintained as follows: (2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperture controls shall be maintained to automatically regulate the temperture of hot water used by residents fo attain a temperture of not less than 105 degree F(41 degree C) and not more than 120 degree F (49 degree C) Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in 3 out of 3 bathroom sinks delivered water above 120F ( 136F, 134F and 124 F) which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 01/07/2026 Plan of Correction Licensee will lower the water temperture to meet Tittle 22 requirements ( 105-120F) and send to Licensing via the office Fax, (323) 980-4912 a 3 day water log for all 3 sinks. Water was lowered during visit. Log will be sent for the next 3 days.

Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above as during facility tour LPA observed water temperature in kitchen to be 139.8 degrees F (with no signs indicating water temp is over 120) and water temperature in main resident restroom measued at 131.1, which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 12/19/2023 Plan of Correction **Staff lowered water temperature during visit today** Licensee/Administrator will create a water log starting 12/19/2023 and record water temperature readings three times daily (morning, day, evening) for the next 5 calendar days, ending on 12/23/2023. Licensee/ Administrator will send proof of water temperature log to LPA via email by 12/24/23. First record of water log on 12/19/23 and subsequent recorded readings must measure within 105 -120 degress F, with date and time of reading.
