The licensee shall ensure that residents are regularly observed for changes in physical, mental, emotional and social functioning and that appropriate assistance is provided... This requirement was not met evidenced by: Based on interviews and record review, S1 had not completed a re-appraisal for R1 since their admission into the facility which poses a potential health and safety risk to persons in care.
