What the official deficiency says

(a) The licensee shall ensure that a separate, complete, and current record is maintained for each resident in the facility or in a central administrative location readily available to facility staff and to licensing agency staff. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview, record review, the Administrator did not comply with the section cited above in that based on observation, the bedtime medications for (1) resident was administered on the evening of 02/16/2026, however, the staff failed to sign/initial the Medication Administration Record (MAR), which poses/posed an immediatel health, safety or personal rights risk to residents in care.

POC Due Date: 02/18/2026 Plan of Correction Administrator shall provide in-service training to all staff on how to properly document the Medication Administration Record (MAR), as well as develop a policy requiring that (2) people verify medication records. Administrator to send a copy of the training log, along with the topics covered, and a sign-in sheet of staff who participated to CCL/LPA by POC due date.

What the official deficiency says

The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provisions of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement is not met by evidence of: LPA and S1 observed living and dead roaches in the kitchen cabinets and surrounding area.

Administrator will contact the licensee and ask for treatment of the roach infestation. Administrator will send copy of invoice or proof that professional pest control has rendered services and send proof to LPA by POC dated.

All facilities shall have telephone services on the premises. This requirement is not met by evidence of: LPA attempted to call on 01/10/2023 and phone was not working. 2 staff and administrator stated phone was not working for some time.

Personal Rights of Residents in All Facilities To leave or depart the facility at any time and to not be locked into any room, building, or on facility premises by day or night. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation during physical plant inspection, LPA observed an exit side gate on left side of the facility was a turn knob but was locked with a key lock. It required a key to unlock. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 03/26/2022 Plan of Correction Key lock on the exit side gate was removed during the visit. Licensee to ensure that all exit gates shall remain unlocked at all times. Administrator agrees to submit a self-certification to ensure that all exit gates shall remain unlocked at all times to Licensing by POC due date.
