(5) Under no circumstances shall postural supports include tying, depriving, or limiting the use of a resident's hands or feet. (A) A bed rail that extends from the head half the length of the bed and used only for assistance with mobility shall be allowed. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview and record review, the licensee did not comply with the section cited above in [1] out of [6] resident #1 bed has a half rail which does not have a physician order and resident #1 is not on Hospice, which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 10/19/2025 Plan of Correction Administrator/Licensee will send LPA a plan indicating the request made to R1's physician for the half rails by POC due date 10/19/25. Licensee will send LPA, proof of physician order for half hand rail for R1's bed once it is approved.

Bed rails that extend the entire length of the bed are prohibited except for residents who are currently receiving hospice care and have a hospice care plan that specifies the need for full bed rails. This requirement is not met as evidenced by: Based on observation, resident R2 has two (2) half rails on their hospital bed, converting it to full rails. R2 is not enrolled in hospice care services, which poses an immediate health, safety or personal rights risk to resident in care.

Licensee shall ensure that all persons prior to working at the facility are associated through Guardian system. A civil penalty was assessed. Assistant Administrator associated S1 via Guardian during the visit. ***Cleared.

(a) All facilities shall maintain a fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. Prior to accepting or retaining any of the following types of persons, the applicant or licensee shall notify the licensing agency and obtain an appropriate fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. (2) Bedridden persons. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and record review, the licensee did not comply with the section cited above. The facility has 2 bedridden residents, but only licensed for 1 bedridden, which poses an immediate health, safety or personal rights risk to persons in care.
