What the official deficiency says

(a) A plan for incidental medical and dental care shall be developed by each facility. The plan shall encourage routine medical and dental care and provide for assistance in obtaining such care, by compliance with the following: (4) The licensee shall assist residents with self administered medication as needed. ***This was not met as evidenced by it was reported that there were pills in R1's bedroom, when found they were provided to staff 2 (S2). During visit on this date, resident 2 (R2) provided a pill to LPA that was found at dining room table belong to resident 3 (R3).

(a) For any rate increase due to a change in the level of care of the resident, the licensee shall provide the resident and the resident’s representative, if any, written notice of the rate increase within two business days after initially providing services at the new level of care. The notice shall include a detailed explanation of the additional services to be provided at the new level of care and an accompanying itemization of the charges. ***This was not met as evidenced by Responsible party for R1 was notified verbally on 3/22/26 and via text message on 3/30/26 of a rate increase effective 4/01/26. 87309 Storage Space and Access (a)... licensee shall ensure that, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This requirement was not met as evidenced by: Licensee did not ensure that items which could pose a danger to residents were locked or stored inaccessible to residents. A chef cooking knife was left unattended on the kitchen counter while residents were present. This poses an immediate health and safety risk to persons in care.
