87705 Care of Persons with Dementia (c) Licensees who accept and retain residents with dementia shall be responsible for ensuring the following: (5) Each resident with dementia shall have an annual medical assessment...and a reappraisal done at least annually... This requirement is not met as evidenced by: During records review, LPA found that R1 has dementia and the last obtained medical assessment was completed on 1/13/17, and the facility has not completed an annual reappraisal assessment of R1 and does not have records of any prior reappraisals completed, which poses a potential health risk to residents in care.

87303 Maintenance and Operation (c) All window screens shall be clean and maintained in good repair. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. The window screens in bedrooms #1,2, and 3 are all bent, covered in dust/dirt, and in disrepair, which poses a potential health, safety or personal rights risk to persons in care.
