What the official deficiency says

(b) The following food service requirements shall apply:(21)...refrigerators of adequate size shall maintain a maximum temperature of 40 degrees F (4 degrees C)... This requirement was not met evident by: Based on LPA Williams observation, the refrigerator temperature could not reach a temperature lower than 53 degrees F. which poses a potential health and safety risk to persons in care.

Hall bathroom shower faucet observed with water leaking and sliding door observed not able to be opened or closed with ease, which poses a potential health, safety or personal rights risk to persons in care.

87705 Care of Persons with Dementia (f) The following shall be stored inaccessible to residents with dementia: (1) Knives, matches, firearms, tools and other items that could constitute a danger to the resident(s). This requirement is not met as evidenced by: Deficient Practice Statement Based on observations, the licensee did not comply with the section cited above. LPA observed padlock for cabinet under kitchen sink was hooked into latch, but was not locked and was accessible to the two cans of Comet cleaner inside the cabinet, which poses an immediate safety or personal rights risk to persons in care.
