A licensee shall notify the department, the State Long-Term Care Ombudsman, all residents, and, if applicable, their legal representatives, in writing, within two business days of filing for bankruptcy. This requirement was not met as evidenced by: The department was informed that the Licensee filed Chapter 11 Bankruptcy on May 21, 2024 and the Licensee did not notify the Department within a 2-day period. This poses a potential health and safety risk to residents in care.

Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement has not been met as evidenced by: Based on department’s facility walkthrough, facility appeared to have a leak in the living room that is in need of repair and a bed located in an office that is not designated as a bedroom. This poses an potential risk to residents in care.
