This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above LPA observed alteration on garage, licensee stated that they added additional room and change the garage flooring on January 2022 without informing city planning or CCL office which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 05/27/2022 Plan of Correction Licensee agreed remove furnishings in garage living area and instruct staffing to not reside in the garage living area until city planning gives instrction to licensee if new bedroom is permitted. Licensee also agreed to to obtain permit with the City Planning Office for alteration in the garage , LIC200 for request for fire inspection and submit updated sketch to LPA by 5/27/2022.
