(k) The licensee shall maintain documentation of criminal record clearances or criminal record exemptions of volunteers that require fingerprinting and non-client adults residing in the facility. (1) Documentation shall be available at the facility for inspection by the Department. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above by not having S3's file at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

(a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above by leaving chemical cleaner unlocked and accessible to residents which poses an immediate health, safety or personal rights risk to persons in care.
POC Due Date: 06/27/2025 Plan of Correction Facility agrees to lock the laundry room door and review regulations and submit a letter of self certification to CCLD by POC date.
(b) The following food service requirements shall apply: (26) Supplies of nonperishable foods for a minimum of one week and perishable foods for a minimum of two days shall be maintained on the premises. POC Due Date: 07/09/2025 Plan of Correction The facility agrees to buy additioanl food, both parishable and non parishable. Proof of correction will be sent to CCLD by POC date.
(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1569.17(b) shall prior to working, residing or volunteering in a licensed facility: This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in by not having S3 fingerprint cleared and associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.
