(e) Water supplies and plumbing fixtures shall be maintained as follows: (5) Slip-resistant mats, strips, or flooring shall be used in all bathtub and shower floors. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in not having slip resistant mats, strips, or flooring in two (2) out of three (3) resident's shared restroom which poses a potential health, safety or personal rights risk to persons in care.

Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in having two knives in an unlocked kitchen drawer which poses an immediate safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in having Cutter & Repel Sportsman- Insect Repellent, Swan- Rubbing Alcohol and Marquee- Hydrogen Peroxide in an unlocked cabinet inside laundry room which poses an immediate health, safety or personal rights risk to persons in care.

Criminal Record Clearance. All individuals subject to a criminal record review...prior to working, residing or volunteering in a licensed facility: Obtain a California clearance... This requirement is not met as evidenced by: Deficient Practice Statement Based on interview and records review licensee failed to comply with the above, licensee let S4 started working at the facility without obtaining fingerprint clearances, records review indicated that S4's fingerprint clearance " still in process " , which poses an immediate health and safety risk to residents in care.

POC Due Date: 09/07/2022 Plan of Correction Administrator sent S4 home. Adminstrator agreed that S4 will have fingerprint cleared before working at the facility. Civil penalty was assesed during the visit. Cleared during the visit.
